Barcode Assistant by Apodidae is a very nice reader. Works well, and it has all the options (go to a web site, save, make a call or send a SMS, e-mail codes, import vCard entries, etc.)
Tested on several QR codes, with good success - it decodes, then gives you the appropriate options, in this case to go to the site or to save it, see below:
Beyond being a good QR code reader, you can also generate QR codes. You can use the 'Quick Create' function to generate QR codes "by hand" - web urls, e-mail addresses, phone numbers, SMS, and even your location per the BB GPS (the "Geo")! And the software shines with its integration with the Blackberry PIM to allow you to create QR codes from your contacts, appointments, or the web browser.

Then you can pull up your history, and have the various possibilities (e-mail, etc.) available to you...
The bottom line? Highly recommended, one of the better QR readers available at this time! A definite "keeper."
